Read that list carefully and a pattern emerges. Hormone therapy improves the conditions under which weight loss becomes possible. It protects muscle and influences where fat is stored. Those are meaningful benefits, and for many women they are the difference between a plan that works and a plan that collapses by week three.
What BHRT Cannot Do
This is the part that honest medical marketing often skips.
It is not a weight loss medication. Hormone therapy is not FDA approved or indicated for weight loss or obesity treatment, and it should not be prescribed or sold on that basis.
It will not produce the kind of loss that changes health markers on its own. Clinically meaningful weight management generally means a reduction of five to ten percent of body weight. The weight effects of hormone therapy fall well below that range.
It does not replace nutrition, activity, or strength training. Muscle is preserved by using it. No prescription substitutes for resistance training twice a week.
It does not override an untreated thyroid problem, insulin resistance, or a medication that is driving weight gain. Those need to be identified and treated on their own terms.
It is not appropriate for everyone. Pregnancy, unexplained vaginal bleeding, a history of breast, uterine, or endometrial cancer, prior stroke or heart attack, blood clots, and liver disease all require a provider to review whether hormone therapy is suitable.
A provider who promises pounds from a hormone prescription is telling you something the evidence does not support. A provider who explains what hormone therapy protects, and then builds the weight loss plan separately, is doing the job properly.
Bioidentical Does Not Automatically Mean FDA Approved
This distinction is worth understanding before any appointment, because the word “bioidentical” is used in two different ways.
FDA-approved bioidentical products exist and are widely prescribed. Estradiol and micronized progesterone are chemically identical to the body’s own hormones, and products such as Prometrium and Bijuva are approved, standardized, and regulated for potency and purity.
Compounded bioidentical hormone therapy is different. These preparations are mixed at a compounding pharmacy and are not FDA approved. A National Academies review summarized by the FDA found a lack of rigorous evidence for the safety and effectiveness of compounded preparations, and recommended restricting their use to specific circumstances, such as a documented allergy to an ingredient in an approved product or a dose that is not commercially available.
Compounded hormones have a legitimate place. They are simply not the default, and you are entitled to ask which category your prescription falls into and why that choice was made for you.
The Role of Blood Work and Hormone Testing
Symptoms point in a direction. Lab work tells you whether hormones are actually the problem, whether something else is contributing, and whether a given treatment is safe for you.
A baseline panel before hormone therapy or weight loss medication generally includes:
Hormone testing. Estradiol, progesterone, FSH, and testosterone to establish where you are in the transition and whether levels match symptoms.
Thyroid evaluation. TSH and related markers, because an underactive thyroid produces fatigue, weight gain, and brain fog that look almost identical to menopause.
Liver function testing. Important before starting any medication the liver processes.
Comprehensive metabolic panel. Kidney function, electrolytes, and fasting glucose.
Hemoglobin A1C. Screens for insulin resistance and prediabetes, both of which make weight loss harder.

Updated labeling also reflects evidence that women who begin hormone therapy within ten years of the onset of menopause see more favorable long-term outcomes.
A label change is not a blanket clearance. It shifts the decision back where it belongs, which is an individualized assessment between a patient and a licensed provider. Your evaluation should still cover personal and family history of breast, uterine, and endometrial cancer, cardiovascular history including stroke, heart attack, and clotting disorders, liver health, bone density risk, and how far you are from your final menstrual period. Where Weight Loss Medication Fits Alongside Hormone Therapy

Does BHRT cause weight gain?
